Card Issuance

Each employee will receive identification cards at his or her home address in the weeks following the submission of an enrollment application. Identification cards must be presented each time an insured receives medical, dental, vision, or pharmacy services.

You may order additional cards online or by calling the Highmark Dedicated Customer Service Unit at (800) 325-4560.

Identification Cards

SelectBlue ID Card

Each employee and each dependent enrolled in SelectBlue will receive his or her own plastic medical/prescription identification card. This card closely resembles a credit card and should be presented each time services are performed. The Highmark Blue Cross Blue Shield, SelectBlue, and Premier logos appear on the top of the card. The name listed on the card will be that of the covered individual. To protect your privacy, a unique member identification number is listed on the card instead of a social security number. The PCP's name and telephone number are listed, as well as the effective date of the PCP designation. The office visit co-payment amount appears in the lower left-hand corner.

CommunityBlue ID Card

Each employee and each dependent enrolled in CommunityBlue will receive his or her own plastic medical/prescription identification card. This card closely resembles a credit card and should be presented each time services are performed. The Highmark Blue Cross Blue Shield, CommunityBlue and Premier logos appear on the top of the card. The name listed on the card will be that of the covered individual. To protect your privacy, a unique member identification number is listed on the card instead of a social security number. The PCP’s name and telephone number are listed, as well as the effective date of the PCP designation. The office visit co-payment amount appears in the lower left-hand corner.

KeystoneBlue ID Card

Each employee and each dependent enrolled in KeystoneBlue will receive his or her own plastic medical/prescription identification card. This card closely resembles a credit card and should be presented each time services are performed. The Keystone Health Plan West, KeystoneBlue, and Premier logos appear on the top of the card. The name listed on the card will be that of the covered individual. To protect your privacy, a unique member identification number is listed on the card instead of a social security number. The PCP's name and telephone number are listed, as well as the effective date of the PCP designation. The office visit co-payment amount appears in the lower left-hand corner.

PPOBlue ID Card

Each employee and each dependent enrolled in the PreferredBlue PPO program will receive his or her own laminated medical/prescription identification card. This card closely resembles a credit card and should be presented each time services are performed. The Highmark Blue Cross Blue Shield, PreferredBlue and Premier logos appear on the card. The name listed on the card will be that of the covered individual. To protect your privacy, a unique member identification number is listed on the card instead of a social security number. The office visit co-payment amount appears in the lower left-hand corner.

Bluecard Out of Area PPO ID Card

Each subscriber in this program will receive two laminated medical/prescription identification cards. This card closely resembles a credit card and should be presented each time services are performed. The "PPO in a suitcase" logo appears in the upper right hand corner and identifies the subscriber as a BlueCard member. To protect your privacy, a unique member identification number is listed on the card instead of a social security number.

Traditional ID Card

Each subscriber in this program will receive two paper identification cards. Both cards will list the name and identification number of the employee. The Highmark Blue Cross Blue Shield logo appears at the top of the card. Any supplemental lines of coverage (i.e. dental, vision) the insured is enrolled under the same group number will also be listed on the card. Employees and their dependents should present this card each time services are rendered.

Out-of-Area Comprehensive ID Card

Each subscriber in this program will receive two paper identification cards. Both cards will list the name and identification number of the employee. The Highmark Blue Cross Blue Shield logo appears at the top of the card. Any supplemental lines of coverage (i.e. dental, vision) the insured is enrolled under the same group number will also be listed on the bottom of each card. Employees and their dependents should present this card each time services are rendered.

65 Special

Each subscriber in the program will receive two paper identification cards. Both cards will list the name and identification number of the employee. The Highmark Blue Cross Blue Shield logo appears at the top of the card. This card and should be presented each time services are performed along with the subscriber’s Medicare ID cards.

Dental and Vision ID Cards

Each subscriber will receive two paper identification cards. Both cards will list the name and identification number of the employee. The United Concordia logo appears at the top of the dental cards while the Highmark Blue Cross Blue Shield logo appears at the top of the vision cards. Employees and their dependents should present this card each time services are rendered.

Dental Plus participants will receive a separate, laminated paper card.
