Dental Benefits Overview

Together the Council and Highmark Blue Cross Blue Shield have created five dental options designed to improve the quality, delivery and affordability of dental care. Subscribers in the Concordia Flex Dental Options I through IV are encouraged to use Concordia Advantage participating dentists. Dual dental options are available for groups with 25 or more employees.
Subscribers in PennDental Options I through IV are encouraged to use Blue Cross Blue Shield participating dentists. Dual PennDental options are available for groups with 25 or more employees.
The ConcordiaPLUS Dental HMO is a completely voluntary program and requires members to select a Primary Dental Office (PDO) which will provide, or arrange for all eligible dental care. The ConcordiaPLUS Dental HMO may be offered along with any PennDental Option.

Requirements
Table below shows the current minimum group size for
companies to enroll in any of Highmark's Blue Cross Blue Shield Dental Programs.
Please note that companies with more than 25 employees may offer any combination
of a designated base and optional plans.

Provider Network
Members can use the Provider Network Directory provided by United Concordia, Inc. to search for participating dentists.
Rates
The rates listed below include a 3% administrative fee and are guaranteed to December 31, 2004.
