Following is a summary of the coverage level, terms and conditions for the Concordia PLUS Dental HMO Plan offered by the Council's Employee Benefits Group. ConcordiaPLUS consists of a limited network of dental providers and a primary dental office must be chosen.

All percentages are the percent of United Concordia's maximum allowable charge (MAC). All members will receive a copyament schedule that shows the financial obligation for which he or she will be responsible under this program.
